W9 Forms
Indian Prairie School District #204 requests that all officials working athletic events
at any of the middle schools or high schools fill out a W-9 form and have on file at the
district office. This will be requested every year. In respecting the privacy of the
officials and confidentiality of social security numbers we ask each Official to return
the completed W-9 form to:
